The practice is happy to accept new patients. You will need to live within the practice catchment area (see map page).
To register you will need to complete a registration form (click here to download Patient Registration Form) and Patient Health Questionnaire form (click here to download the Patient Health Questionnaire) alternatively these are available from reception staff at the practice. Once completed you will need to bring them to the practice along with proof of residency. This can be verified by a recent utility bill, or other official correspondence eg bank statement, letter from a government agency (Pensions Agency, Home Office, DWP or HM Revenue and Customs) with your current address on it.
All new patients registered will be allocated a "named doctor". You can choose to see any doctor when you need an appointment it does not need to be your named doctor.
Temporary Residents are patients who may be staying in the area visiting family and friends or on holiday that are taken ill and need to be seen by a doctor. We will need some details from you such as your current doctor's details and your temporary address. You will need to complete a Temporary Residents for which you can download (click here to download the Temporary Patient Registration form) or alternatively please come to the surgery 15 minutes before your appointment to complete one.
In most cases these patients need to be seen urgently and we would therefore ask you to telephone as early as possible. It is likely you will be seen by our nurse Practitioner in one of our Urgent Care Clinics.
